Beloved father of Henry Kooistra and his wife Tina of Stroudsburg, PA, Barbara Maylath and her husband Richard of Collingswood, Kevin Kooistra and his wife Julie of Saddle Brook, step-daughters Lori Wenzel of Paramus, Lisa Oglesby of Weatherford, TX. Beloved grandfather of Brandon and his wife Jessica, Emily, Katie, Jimmy, Carly, Alex, Griffin and Jordan. Beloved great-grand-father of Emma. Loving brother of Sue Jacklin and Janet Decker and her husband George. Pre-deceased by his brother M. William Kooistra and his sisters Katherine Faul, Georgette Du Bois, Ruth Ferri and Evelyn Barron.
